Professional Experience

Joined Triangle J previously as Options Counselor serving residents of skilled nursing facilities interested in transitioning back into the community, and prior to that, Project Coordinator for Triangle Agencies Anticipating Disasters. Rejoined Triangle J late 2016 as Community Resource Connections (CRC) Coordinator for Wake County; also coordinates MIPPA activities for the region. Has 20 years of experience in working with older adults and with individuals with special needs and their families in a variety of positions from direct service to program management. Served as Eldercare Consultant, has provided care management services to individuals and their families, education and training, and project consulting to senior living communities, health care organizations, and faith communities. 

 

Responsibilities

  • Coordination of CRC network activities
  • Information regarding available services for older adults, persons with disabilities, and their caregivers
  • Development of annual work plan for the Wake CRC
  • Recruitment of new CRC partner agencies
  • Education and training for CRC and other community partners
  • Coordination and reporting for Triangle J regional MIPPA activities
